However, relatively little is known about the role of histopathology in the study of PCa at the molecular level. The molecular genetics of histopathological processes, also called transcriptome, have helped to identify some molecular processes that are relevant to PCa. The former are significantly altered in POC but in various cancer types, thus providing a clue to elucidating their regulation in different stages, and thus suggesting for further research into understanding the molecular mechanisms of PCa development ([@B1],[@B2]). Nevertheless, there are many challenges to continue to unravel the molecular mechanisms responsible for the functional changes of PCa cells: 1. The expression levels of proteins involved in proteolytic and autoclaving steps, which have been termed “matrix break” have been evaluated. These findings suggest changes at the proteolytic pathway to a more diverse array of matrix proteins, as well as to a non-classifiable rate of proteolysis ([@B3]). 2. A hallmark of this nonclassifiable rate might be the possible alteration of the epithelial-mesenchymal transition, which is affected by several mechanisms that involve changes of proteins at the cellular membrane. 3. Also the expression of proteins involved in calcium regulation, which are not under the control of the tumor suppressor protein p21, and the degradation of the focal adhesion at the tumor field ([@B4]). The molecular mechanisms of these changes include: 1. Both tumor suppressor and focal adhesion kinase expression is altered at the molecular level, resulting in changes at calponin 1 (CXCR-N) cell surface ([@B5]). 2. Calcium at the focal adhesion complex is differentially regulated by different molecular pathways. What is the role of histopathology in the study of prostate cancer? The purpose of this paper is to highlight interesting aspects that have been uncovered recently, that is, those concerning microvascular hyperpermeability, tubuloalveolar lesions, and lymphocytic differentiation. In part I of this update, we re-analyse a growing body of research that shows the remarkable importance of histopathological observations in the preclinical diagnostic process. We also suggest that it may be useful for preclinical diagnostic purposes, an important first step in the development of biomarkers, disease prediction procedures, and therapies. This review would benefit knowledge, education, planning, and management for the use of histopathology in preclinical disease diagnostics as important future challenges both to advance diagnostic and prognostication priorities and to optimize local and systemic treatment options. Despite the excitement regarding this potential and an active dialogue, recent advances in histopathology – technology, research methods, models for disease diagnosis, and patient care – has been lacking.

We are taking advantage and extending a career range of research efforts to assess potential impact of increased clinical diagnostic imaging technology during the past decade. This paper addresses some of these challenges, including its relevance, contributions, and potential impact for preclinical diagnostic imaging in health and disease prevention, and the development of new biomarker features that promote successful preclinical diagnosis. While this review is yet to find any beneficial advancement in preclinical imaging technology, certain important aspects of the research reported are being identified and suggested. Through our expertise, we will play a major role in designing proper protocols, including an optimal experimental setup for testing biomolecules; post-processing, and post-processing evaluation, and ensuring full reproducibility and reproducibility, with immediate effect on the ultimate clinical picture.
